Care Central VNA & Hospice, Inc offers our patients diversity in our service lines. Our goal is to help patients receive the care they need where they live, whether it’s nursing, therapy, end-of-life care or private care. We want to reassure you that you are not alone; our Hospice experts can help guide you through every step when dealing with a life-limiting illness. Living with a serious illness can be overwhelming for both patients and caregivers. Our Palliative Care Bridge Program provides an extra layer of support for individuals and their families, who are receiving home health services through Care Central VNA & Hospice, Inc.
This program focuses on improving quality of life by helping manage pain, symptoms, and emotional stress. Palliative care is available at any age and at any stage of a serious, life-limiting illness, and can be provided alongside curative treatments, offering patients and families greater comfort, guidance, and support during the course of their illness
Adult day health care is a cost-effective means to help elderly and disabled adults maintain their independence in their homes, as well as in the community. Our medical model day center is designed for the comfort and care for your loved one in a safe, uplifting, and home-like environment
Healthy Families is a home-based family support and coaching program that supports first time parents aged 23 and under to create stable, nurturing environments for their children. Healthy Families is a program of Children’s Trust and is implemented locally by Care Central VNA & Hospice, Inc.
As part of Care Central VNA & Hospice’s commitment to a healthy community, we opened a Diaper Pantry on May 1, 2014. Care Central VNA & Hospice, Inc believes that a healthy community includes our youngest residents.
A health management system to enable patients to stay in their own home, while their symptoms are closely monitored to avoid hospitalization.
Insurance
We accept patients with insurance coverage through Medicare, Medicare Advantage, Medicaid, Private Insurance, most HMO’s and Private Pay. If you have any questions regarding coverage or a specific plan, please contact us.
