Provides close symptom monitoring of many conditions that transmits daily vital signs and other clinical information directly to Care Central VNA & Hospice. This allows for early intervention in response to changes in a patient’s condition that may occur outside of Care Central VNA & Hospice clinician’s visit.

Provide exercises, activities and functional training designed to restore physical function and optimize independence. Therapists evaluate the need for assistive equipment to promote the highest level of function within the home and community while ensuring safety.

Offer practical counseling for patients and their families concerning emotional, economic or other issues related to their illness. They also refer clients to other specialized services and assist in long-term placement and coordination of community resources.
