A health management system to enable patients to stay in their own home, while their symptoms are closely monitored to avoid hospitalization.

Care Central VNA & Hospice currently provides symptomatic and biometric monitoring via our Telehealth Program for our clients experiencing:

  • Diabetes
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Heart Failure
  • High Blood Pressure
  • Coronary Artery Disease

Our telehealth system provides critical healthcare data quickly to both patients and their physicians. This better, faster, and more specific information identifies subtle changes in condition before symptoms go undetected and a client’s health is at risk. The system is a tool proven to reduce unnecessary hospitalizations and improve health and wellness.

Please note: The Telehealth Program does NOT replace home visits by your nurse or therapist. It is intended to enhance the care provided and is used as a complement to our direct care service to astutely monitor symptoms outside of home visits.
